Most kindergartners are up to date with their vaccines, but federal immunization goals nevertheless remain unmet, the CDC says. According to a CDC report, the average number of kids who have been vaccinated for measles , mumps, and rubella is 94.8%. Health officials have set their target at greater than 95%. ?Coverage is not the way it should be,? says Jaime Deville, MD. Deville is a professor of pediatric infectious diseases at UCLA?s Mattel Children?s Hospital. He reviewed the report for WebMD. ?This report really confirms what?s already well known,? he says. The report?s authors point out that vaccine rates vary significantly from state to state. The five states that are doing the best job of vaccinating kids against measles, mumps, and rubella (MMR) are: Texas 99.3% Mississippi 99.2% Nebraska 99% Maryland 98.7% Louisiana 98.1% The five states that need the most improvement when it comes to MMR vaccines are: Colorado 86.8% Pennsylvania 86.9% Kansas 88.2% Idaho 89.2% North Dakota 90.6%? Tetanus, Diphtheria, and Other Vaccines The report also tallies vaccine numbers for tetanus , diphtheria, acellular pertussis (whooping cough), polio, hepatitis B, and varicella (chickenpox). Forty seven states, the District of Columbia, and five other areas reported their vaccine numbers. The city of Houston submitted its own figures, which were the highest reported, at 99.5% for MMR. Figures for Wyoming, New Hampshire, and New Jersey were not available. California, the state where Deville teaches and practices, met the 95% goal for only two of the vaccines: chickenpox and hepatitis B. ?Our coverage is really not ideal,? says Deville. He says the number of children in his state who were granted exemptions from vaccines for medical, religious, or philosophical reasons is too high for comfort. ?We see there are 12,000 out of the half million kindergarten age kids here,? says Deville. ?That?s a sizable percentage that right off the bat won?t be vaccinated.? Exemptions From Immunization Overall, according to the report, 1.5% of children were granted exemptions from vaccinations. That figure varied dramatically from state to state. Mississippi, which does not allow religious or philosophical exemptions, had an exemption rate below 0.1%, the lowest number in the country. Alaska granted exemptions to 7% of its children. Arkansas reported the largest increase in exemptions from the previous year; Nebraska reported the steepest decline. For the most part, the percentages reported reflect statewide averages. However, the report?s authors point out the need to focus on local rates of immunization that might get overlooked when focusing on the big picture. ?Since exemptions cluster geographically, there might be smaller areas and schools where low levels of immunization could sustain ongoing measles transmission after importation from other countries,? the report states. Measles Still Threatens Children?s Health While the current level of vaccine coverage is likely to prevent any major outbreaks of measles, Deville says, measles ? and other diseases for which vaccines are available ? should be on parents? radar. ?We?ve been seeing only about 50 cases a year for the past several years, but that number has begun to creep up,? he says. In 2011, 222 cases of measles were reported to the CDC, the highest number in 15 years. Most of those cases, the report says, were brought here from outside the U.S. ?Measles,? Deville says, ?is one of the most contagious diseases, with significant complications and mortality. We don?t want to deal with it again.? Deville points to recent whooping cough outbreaks as examples of why parents need to make sure their kids are properly vaccinated. It?s easy to think there is little to worry about when such diseases are rarely seen these days, he says. That?s a mistake. ?Just because we don?t see them doesn?t mean they no longer exist,? Deville says. ?These diseases are not gone.? Abstract

PURPOSE:

The World Cancer Research Fund/American Institute for Cancer Research identified a probable role for garlic in colorectal cancer prevention?based?on preclinical?evidence?and epidemiologic studies, but prospective data are limited. The purpose of this paper was to contribute additional?evidence?on this topic for men and women in a large prospective cohort study.

METHODS:

In 1999, 42,824 men and 56,876 women in the Cancer Prevention Study II Nutrition Cohort completed a questionnaire with information on dietary garlic consumption. Garlic supplement use was assessed in 2001. Cox proportional hazards regression was used to estimate multivariable-adjusted hazard rate ratios (HRs) and 95?% confidence intervals (CIs).

RESULTS:

During 7?years of follow-up, 579 men and 551 women were diagnosed with colorectal cancer. Among men, daily garlic consumption was associated with a non-significant higher colorectal cancer risk (HR?=?1.04, 95?% CI 0.99-1.08 for each additional clove or "4 shakes" of garlic per week), whereas the association was borderline inverse in women (HR?=?0.95, 95?% CI 0.91-1.00, p heterogeneity by sex?=?0.03). Garlic supplement use was not related to a lower risk of colorectal cancer, and in men, former use was associated with a higher risk of colorectal cancer (HR?=?1.85, 95?% CI 1.13-3.03).

CONCLUSIONS:

These results provide weak support for a role of dietary garlic consumption in colorectal cancer prevention in women, but a possible increased risk in men. Further research is needed to confirm different associations by sex.

Explosion kills 39 at Venezuela's biggest refinery

PARAGUANA/CARACAS, Venezuela (Reuters) - An explosion tore through Venezuela's biggest refinery on Saturday, killing 39 people, wounding dozens more and halting operations at the facility in the worst accident to hit the OPEC nation's oil industry.

Energy Minister Rafael Ramirez told Reuters no production units at the Amuay refinery were affected and that there were no plans to halt exports, a sign that the incident will likely have little impact on fuel prices.

Photographs taken shortly after the pre-dawn blast showed wrecked vehicles, flattened fences and giant storage tanks buckling and crumpling as flames lit the night sky. A National Guard building in the area was shattered and officials said a 10-year-old child was among the dead.

A gas leak caused the explosion and many of those killed were National Guard troops who were providing security for the 645,000 barrel-per-day facility, Ramirez said, adding that the fire was under control.

Officials said 18 of the soldiers were confirmed dead along with 15 civilians, while six bodies still had to be identified.

"There was a National Guard barracks near the explosion. ... The installation was too close to the operations," Ramirez told Reuters in an exclusive telephone interview, adding that production could resume at Amuay within two days at most.

"We need to boost production at other refineries and look for floating storage near the complex," he said.

The explosion follows repeated accidents and outages during the last decade across installations run by state oil company PDVSA that have limited output and crimped expansion plans.

Amuay has partially shut operations at least twice this year due to a small fire and the failure of a cooling unit.

Those problems have spurred accusations of inept management by the government of President Hugo Chavez, who is running for re-election on October 7. Acrimony over the explosion could spill into an already bitter campaign, but is unlikely to overtake larger political concerns such as crime and the economy.

"I want to convey the deepest pain that I've felt in my heart and soul since I started to get information about this tragedy," Chavez said in a phone call to state TV. He declared three days of mourning.

FIRE UNDER CONTROL

Venezuela has traditionally been a big supplier of fuel to the United States and the Caribbean, but refinery shutdowns have become so common that they rarely affect market prices.

Traders told Reuters the docks at the refinery were shut, and tankers were anchored offshore waiting. They said this would cause delays to some of the country's exports.

The explosion broke windows at homes in the area, a peninsula in the Caribbean sea in western Venezuela, as well as at Amuay's main administrative building.

The blast was also felt out at sea in the Paraguana bay, where some crew members on moored tankers were knocked off their feet by the shockwave, one shipping source said.

Ramirez said the fire that started after the explosion had affected only nine storage tanks holding mostly crude oil and some processed fuels including naphtha.

Officials said two tanks were still burning off residual fuel, and a Reuters witness at the scene said large black clouds of smoke hung above the area.

Ramirez said existing fuel stocks around the country were sufficient to guarantee 10 days of exports and local sales. PDVSA has no plans to invoke force majeure, he said, which lets companies stop shipments due to accidents or extreme weather.

Amuay, together with a neighboring facility, forms part of the Paraguana Refining Center, the second-biggest refinery complex in the world, with an overall capacity of 955,000 bpd.

In 2010, there was a massive fire at a PDVSA fuel terminal on the Caribbean island of Bonaire, then a blaze at a dock at the Paraguana complex that halted shipping for four days.

Also in 2010, a natural gas exploration rig, the Aban Pearl, sank in the Caribbean. All 95 workers were rescued safely.

(AP) ? Eager to bolster an inexperienced pitching staff during their first playoff run since 1997, the Baltimore Orioles obtained left-hander Joe Saunders on Sunday in a trade with the Arizona Diamondbacks.

Baltimore gave Arizona right-handed reliever Matt Lindstrom and cash considerations or a player to be named later.

Saunders, who was scheduled to start Sunday against San Diego, was 6-10 with a 4.22 ERA in 21 starts with Arizona this season. He won 12 games last season while helping the Diamondbacks win their first NL West title since 2007.

Baltimore began the day tied for the second American League wild-card spot with Oakland and in third place in the AL East, only four games behind the division-leading New York Yankees.

"I like our ball club's chances," said Dan Duquette, the Orioles vice president of baseball operations. "There's an old saying: If you're going in the right direction, keep walking. So the addition of Joe Saunders gives us a chance to keep walking in the right direction. And I think it gives us better balance against the left-handed lineups that we're going to be facing."

Saunders was born in nearby northern Virginia, played high school ball in Virginia and pitched at Virginia Tech before being drafted in the first round of the 2002 draft by the Los Angeles Angels.

"We've liked Joe Saunders for quite a while," Duquette said. "We had some discussions with him in the offseason. He's a local kid and a professional left-handed pitcher. He's won more than he's lost in the big leagues, he's pitched in the American League, he's pitched in the playoffs. He's pitched in a tough ballpark in Arizona."

The Orioles have only one left-hander in the bullpen and a rotation that includes rookies Miguel Gonzalez, Wei-Yin Chen and Steve Johnson.

"We just liked his experience, which I think compliments our starting rotation," Duquette said of Saunders. "And I also like the fact he's left-handed."

Saunders, 31, has pitched for the Angels and Diamondbacks since launching his big league career in 2005.

"I just like this addition of veteran left-handed depth to our staff," Duquette said. "It helps solidify the starting rotation while we're waiting for (injured) Jason Hammel to return. Then, if Jason Hammel does return, it gives us another good left-handed option because Joe Saunders is very good left-on-left. While we're waiting for (left-handed reliever Troy) Patton to come back, if we have several starters doing well, Joe gives us more depth and experience to our pitching staff."

Lindstrom went 1-0 with a 2.72 ERA in 34 appearances with the Orioles this season. He has held right-handers to a .207 average and scoreless in 10 of his past 11 outings.

Lindstrom has played six major league seasons with Miami, Houston, Colorado and Baltimore.

Duquette believed Saunders would be more valuable to the Orioles than Lindstrom at this point in the season.

"Matt Lindstrom did a nice job for us. He contributed to the bullpen," Duquette said. "I just think the left-handed addition to our starting rotation and the capability to get more innings at a high level was better for the team at this stage of the season. And who knows? Joe Saunders is a local kid. Maybe he's with us beyond this year. That's something we can take a look at. It certainly strengthens us now."

To fill Lindstrom's place on the 25-man roster, the Orioles recalled right-hander Jake Arrieta from Triple-A Norfolk. Arrieta, Baltimore's opening day starter, went 3-9 with a 6.13 ERA in 18 starts with the Orioles before being optioned to Norfolk on July 5.

Newly filed court records allege that the man accused of opening fire on a Colorado movie theater in July told a classmate he wanted to kill people four months before the shooting.

Prosecutors made the contention in a motion released Friday. They are seeking access to James Holmes' records from the University of Colorado Denver's neuroscience graduate program.

Prosecutors wrote that Holmes left the program in June after also making unspecified threats to a professor that month and failing his year-end final.

Holmes' defense lawyer, Daniel King, has said Holmes is mentally ill, setting up a possible insanity defense.

But arguments at a hearing Thursday by Chief Deputy District Attorney Karen Pearson revealed a possible motive: Holmes' anger that he was failing at school, "at the same time he's buying an enormous amount of ammunition, body armor and explosives."

A gag order has been issued in the case. Prosecutors argued that gaining access to the school records would establish a motive by showing what Holmes hoped to accomplish at Colorado University and the "dissatisfaction with what occurred in his life that led to this."

They also want to see records from campus police and a campus threat evaluation team similar to those established across the country after the 2007 Virginia Tech University shootings.
